Memorial Wall Plaque (Inside)
The hand made granite heart Plaques can be sand-blasted with a basic inscription and a motif of your choice.
The lease options include a handmade leaf with gilded letters. For an additional cost the following motif options are available:
Butterfly - Ladybirds - Four Leaf Clover - Acorn - Heart - Cross - White Dove - White Feather - Red Robin - Bumblebee.
Please contact the Bereavement Services Office at the time of ordering should you wish to see examples of the motifs available.
Heart Plaque Examples
The initial Lease Period is 5 years, which is renewable. After 5 years the Plaque will be removed and retained at the Crematorium Office which may be collected by you or a nominated person within 3 months. Plaques not collected will be sent for charitable recycling.

All completed forms can be printed off and send to us by post, or by email to Bramcotecrem@broxtowe.gov.uk.
Once the form has been sent please phone 0115 9173849 to make the payment for the memorial.
